Case Manager resume example

Program directors hiring a case manager need to see the caseload you can carry and what changed for the people on it. Name the population (housing, re-entry, behavioral health, aging, youth), your caseload size and your contact standards. Then count results: housing placements, benefits approved, appointments kept, goals met in service plans. Clean HMIS or EHR records and deadlines met for funders round out the case.

Common mistakes on a case manager resume

  • Never stating your caseload

Fix: Caseload is the first thing a program director wants to compare. Give the number of active clients, the population and how often you saw them, for example "35 adults in permanent supportive housing, monthly home visits and weekly check-ins." If the caseload grew or included high-acuity clients, say so. It shows the pace you can sustain.

  • Listing referrals without follow-through

Fix: Anyone can hand out a phone number. Show that the referral led somewhere: the client enrolled, kept the appointment or got the benefit. A bullet like "Linked 60 clients to primary care, with 48 attending a first visit within 30 days" proves you follow up. That closed loop is what funders and supervisors look for.

  • Ignoring data entry and funder reporting

Fix: Case managers in grant-funded programs are measured partly on records. Name the system you use, such as an HMIS or an agency EHR, and show timeliness and accuracy. Mention any HUD, Medicaid or county reporting you support, and audits you passed. Program managers notice who keeps files complete, because it protects their contracts.

  • Using vague words like "helped clients"

Fix: Replace general help with the concrete task and result: completed a housing application, gathered documents for an SSI claim, arranged transportation to court, set up a payee, enrolled a child in school. Specific tasks show you know the systems your clients deal with. Each bullet should name what you did and what changed for the person.

What should a case manager resume include?

A case manager resume should open with a two or three line summary of your focus, then show each role as results rather than duties. Name the skills the posting asks for, such as Intake Assessment, Service Plan Development and Goal Tracking. Start bullets with verbs like Enrolled, Housed and Linked, and give each one a number where you honestly have one. If you hold a credential such as Certified Case Manager (CCM), Commission for Case Manager Certification (CCMC), list it in its own section.

Case Manager resume bullet examples

The numbers are illustrative. Keep the structure (verb, action, measurable result) and use figures you can back up.

  • Carried a caseload of 36 adults in permanent supportive housing, completing monthly home visits and weekly phone check-ins for 98% of clients.
  • Housed 74 people from shelter in one year through coordinated entry, with 88% still housed at twelve months.
  • Filed 28 SSI and SSDI claims using SOAR methods, with 22 approved on first application.
  • Enrolled 140 clients in SNAP, Medicaid or both within 30 days of intake by gathering documents at the first meeting.
  • Linked 60 clients to primary care, with 48 attending a first appointment within 30 days after reminder calls and bus passes.

  • Prevented 32 evictions in a year by negotiating payment plans with landlords and securing emergency rental assistance.

  • Raised twelve-month housing retention from 79% to 90% after adding a structured 90-day check-in for every new tenant.
  • Trained 8 new case managers on intake, HMIS entry and safety on home visits, cutting their time to a full caseload to 6 weeks.

How to stand out as a case manager applicant

Tailor every application to the posting: the resume, the cover letter and the answers you prepare for the interview.

  • Tailor to the program and population

A housing program, a hospital transition team, a re-entry program and an aging services agency each want different proof. Read the posting for population, caseload and systems. Lead with the matching role, use the posting's terms, such as coordinated entry or care transitions, and move unrelated experience lower on the page. Tailor your case manager resume

  • Use the cover letter to show why the work matters to you

Program directors want people who stay through hard weeks. Open with the population you serve and why, in one or two sentences. Then give one result, such as housing placements or benefits won. Mention any language you speak with clients and your comfort with home visits and driving. Close with your availability. Write a case manager cover letter

  • Prepare for scenario questions

Case manager interviews may ask how you would handle a client who misses three appointments, a landlord threatening eviction, a safety concern on a home visit, or a client in crisis. Practice answers that show you stay calm, follow policy, document and know when to call a supervisor. Have a real example ready for each. Practice case manager interview questions

  • Turn daily tasks into strong bullets

Case managers do dozens of small tasks a day, which makes bullets hard to write. Pick the tasks with a result, add a count and a time frame, and start with a verb such as enrolled, housed or linked. A bullet point generator can give you a first draft for each task. Edit every one to match your real numbers. Generate case manager bullets

Case Manager resume questions

Do I need a certification to be a case manager?

Community case management postings may ask for a bachelor's degree in a human services field and experience rather than a certification. Other employers, such as health plans and hospitals, may prefer or require a credential such as the CCM. CCMC sets eligibility through a qualifying license or a degree in health or human services plus supervised case management experience, so check its current rules.

How is a case manager resume different from a social worker resume?

A social worker resume often leads with a license and clinical skills. A case manager resume leads with caseload and logistics: how many people you serve, what services you connect them to and how fast. If you hold a social work license, list it, but focus your bullets on coordination, benefits, housing and follow-through rather than therapy.

What numbers should I use if my agency does not track outcomes?

Count what you can see in your own records. Caseload size, intakes per month, referrals made and completed, benefit applications filed and approved, housing moves, home visits and court or medical appointments attended are all usable. Round where needed and keep names out. Even a simple count like "filed 40 SNAP applications with 36 approved" shows effort and results.

I am coming from retail or customer service. How do I break in?

Show the parts of the job you already do: handling upset people calmly, tracking many open issues at once, following up on time and keeping records. Add any volunteer, peer support or community work, and a short training such as Mental Health First Aid. Apply for case aide or resource navigator roles if full case manager roles ask for more experience.
